Multi-entity Business Process Flow - missing a step somewhere

I'm trying to create a Business Process Flow that goes across entities (similar to the Lead->Opportunity flow), and I don't seem to have found all the steps yet. I've got a workflow that has Entity A in Step 1 of the BPF, and then Entity B in Step 2. Both have BPFs enabled. That part's easy enough.

I've also written an on-demand workflow (for now, as a test) to create a Entity B record from the Entity A record. Even if I've run that, the second stage of my BPF remains locked, even though every requirement to go to stage 2 is met, and if I click Next Stage it says:

"Select Entity B
No records found"

It will let me create a new Entity B record at that point, but it breaks out of the BPF to do it.  And then after I save it, if I go back to the Entity A record, it still shows the stage as locked, and if I press Next Stage it still says No records found.

I have tried parental and referential relationships between Entity A and B, and neither of them seems to properly make this connect during the BPF (either with manual creation or workflow creation).  I'm sure I'm missing something here, but just can't figure out the last piece of this puzzle...  Any guidance would be appreciated!
